2023 Chancellor Awards for Diversity e-Nomination Form
 
e-Nomination Form Instructions
This form should be completed by the nominator with information about the nominee, nominator, and supporters.   

Endorsement for faculty, staff, residents, and postdoctoral scholars nominations
Endorsement from the nominee's Supervisor and Department's Head is required for the nomination to be considered. You do not need to submit written endorsements but you will need to confirm on this form that you have received the endorsement for the nominee.
*Endorsement for student nominations is not required.
 
Required Nomination Materials
All materials below must be complete and submitted by May 12, 2023 to be considered:
  1. e-Nomination Form - information about the nominee, nominator, and supporter(s). Plus endorsements from the nominee's supervisor and department head.  
  2. e-Nomination Letter Form - citing specific examples of the nominee's contributions as related to the "Selection Criteria" of the award.
  3. e-Support Letter Form - submit up to two support letter citing specific examples of the nominee's contributions as related to the "Selection Criteria" of the award.
